Read our Welcome Letter »Prior to your first meeting you may choose to fill out your paperwork. Simply print out the forms and bring them in filled out to your first session. Also, please email us a front and back copy of your insurance card prior to your visit at intake@phcounseling.com for Family Counseling Center St Pete or phcounselingteam@outlook.com for Palm Harbor Counseling Team, and bring the actual card to your visit. Our therapists each have different plans they are covered under and some do self pay specifically.
Aetna, Baycare, Cigna, ENI, ESI, United Health, United Health, Optum.
When calling your Insurance company to see if we are covered in your network, here are some helpful questions
1. Which therapist in the practice is covered under the tax id/
2. Is my deductible met and if not what is the amount of that?
3. What is my specialist coinsurance and do I need pre authorization?
4. How many visits for outpatient mental health do I have?
